What holistic means in a rehab setting

Holistic programs say they treat the whole person: body, mind and daily habits, as well as substance use. In practice, that usually means a clinical program with extra therapies, such as yoga, mindfulness, nutrition, exercise, massage, art or music therapy, and sometimes acupuncture.

These extras can make a stay calmer and help people learn new ways to handle stress. The question is whether they are added to evidence-based treatment or used in place of it. A program should still offer a medical assessment, licensed therapists, medicine where it helps, and an aftercare plan.

If a program says holistic care replaces medicine for opioid or alcohol use disorder, treat that as a warning sign and ask why.

What the research says

The National Center for Complementary and Integrative Health (NCCIH), part of the National Institutes of Health, has reviewed several of these approaches for substance use disorders (NCCIH summary). In short:

  • Mindfulness may help reduce use of alcohol, cigarettes and opioids. One mindfulness program added to methadone treatment has shown benefits for people with opioid use disorder and chronic pain.
  • Music therapy, added to standard care, may reduce cravings and raise motivation.
  • Yoga has positive early results, but larger, better studies are needed.
  • Hypnotherapy may help, but the findings are not conclusive.
  • Acupuncture research is not consistent enough to support it as a treatment for substance use disorders, though some studies suggest it may ease withdrawal, cravings and anxiety.

NCCIH also advises telling the health care team about any complementary approach, so decisions are made together. Supplements and herbal products deserve extra care, because some can interact with prescribed medicines.

Sleep, regular meals and exercise are part of most holistic programs. They carry little risk and are easy to keep up at home, which makes them worth asking about. Ask how the program helps your loved one build a daily routine that will still work after discharge, in an ordinary house, with an ordinary work week.

Questions to ask a holistic program

  • Who leads each therapy, and what is their license or certification? A yoga teacher and a licensed therapist do different jobs.
  • How many hours a week are clinical therapy, and how many are holistic activities?
  • Do you prescribe medicine for alcohol or opioid use disorder when it is appropriate?
  • Who manages withdrawal, and is a physician available at night?
  • Which practices will my loved one be able to keep up at home?

Luxury and holistic often go together: spa services, chef-prepared food and quiet grounds. Those are pleasant. Ask whether each one is included in the price. To compare settings, see our guides to the Florida Keys, Naples and the Gulf Coast and Orlando and Central Florida.

Holistic rehab questions

Is holistic rehab effective?

It depends on what sits underneath. The holistic parts have mixed evidence. The clinical parts, such as behavioral therapy and approved medicines, have the strongest evidence. A program that does both well can be a good fit.

Is holistic rehab the same as non-12-step rehab?

Not always. Some holistic programs include 12-step meetings, and others use different kinds of peer support. Ask what peer support they offer and how it continues after discharge.

Can holistic treatment help with detox?

Rest, food and gentle movement can make withdrawal more bearable, but they do not make it safe. Alcohol and benzodiazepine withdrawal can be dangerous and needs medical supervision. See medical detox and our at-home detox guide.

Will insurance pay for holistic therapies?

Insurance usually pays toward the clinical part of care. Yoga, massage and similar services are often not covered. Ask for a written breakdown. Our cost guide explains what to ask.

Detox at home, at your hotel, or wherever you choose

Medically supervised detox and rehab where you are staying, run by licensed clinicians. Not right for everyone: severe alcohol or benzodiazepine withdrawal may need a hospital or residential setting.
