PPO plans and luxury rehab
A PPO plan lets you use providers outside its network for an extra cost. How that works for private treatment in Florida, and where the surprises usually hide.

Why families with PPO plans see more options
HealthCare.gov defines a PPO as a plan that contracts with providers to form a network. You pay less when you use providers in the network, and you can use providers outside it for an additional cost.
That second part is why PPO plans come up so often with private treatment. Many smaller or high-end centers are not in every network. With an HMO or EPO plan, which usually pay only for in-network care except emergencies, an out-of-network center may get nothing from your plan. With a PPO, it may get something.
“Something” is the key word. Out-of-network benefits can be much thinner than they look on a summary page. The steps below show where the money actually comes from. For the wider picture, start with our insurance guide.
How the out-of-network math works
Here is the order in which an out-of-network claim is usually worked out. Ask your plan about each step.
- The center’s charge. What the center bills for the service.
- The allowed amount. What your plan decides the service is worth. For out-of-network care this can be far below the center’s charge.
- Your deductible. What you pay before the plan starts paying. Out-of-network deductibles are often separate from in-network ones.
- Coinsurance. Your percentage share of the allowed amount after the deductible.
- Out-of-pocket maximum. The cap on your share in a year. Check whether it applies to out-of-network care at all.
- Balance billing. The gap between the center’s charge and the allowed amount, which an out-of-network center may bill to you.
The last step is where most surprises come from. Ask the center, in writing, whether it will bill you for the balance, and how much it expects that to be.
Questions for your plan and the center
Your plan’s Summary of Benefits and Coverage, a short standard document your insurer or employer can send you, is a good place to start. Then call and ask:
- How do you set the allowed amount for out-of-network residential and detox care?
- Is prior authorization needed for out-of-network treatment?
- Is my employer’s plan self-funded? If so, who makes coverage decisions?
- Do you pay me or the center directly for out-of-network claims?
And ask the center:
- What will you bill my plan, and what will you bill me?
- Do you collect a deposit, and is it refundable?
- Will you give me a written estimate of my share, including any balance bill?
If the gap is too wide, compare paying privately with an in-network option, or look at treatment at home, which is priced differently.
PPO questions
Does a PPO cover luxury rehab?
A PPO may pay toward the clinical part of care at an out-of-network center, after your deductible and coinsurance. It will not usually pay for the comfort side. The only way to know your share is to ask your plan and the center for figures in writing.
Do I need a referral to use my PPO out of network?
HealthCare.gov says PPO members can use providers outside the network for an additional cost. Many plans still require prior authorization for residential or detox care, so ask before admission.
Can I use my PPO for at-home detox?
Possibly, if a licensed provider bills it as medical care. Ask the provider which parts it bills to insurance. Severe alcohol or benzodiazepine withdrawal can need a hospital, and in an emergency, call 911. Our at-home detox guide explains what medically supervised home detox should include.
What is a single case agreement?
A one-time contract between an out-of-network center and your insurer, setting a rate for your loved one’s care. Centers sometimes request one. If they do, ask for the agreed terms in writing before admission.
